Flow of Consultation

Online or Telephone Reservation

Online and LINE reservations are available 24 hours a day. We also accept reservations by telephone.

Arrival and Reception

Please arrive 10 minutes before your scheduled appointment time. Please present your health insurance card at the reception desk.

Filling Out the Medical Questionnaire

Please fill out a medical questionnaire regarding your symptoms and medical history. You can also fill this out online in advance or on your smartphone after arriving at the clinic.

Examination

The doctor will perform a thorough examination and explain the treatment plan according to your symptoms.

Payment and Next Appointment

Please settle your payment at the reception desk and make your next appointment if necessary.
What to Bring

For Insurance-Covered Treatment

  • Health insurance card
  • Medication notebook (if you have one)
  • Medical certificate/subsidy card (if you have one)
  • Referral letter (if you have one)

For Self-Pay Treatment

  • Identification showing your name and date of birth (My Number card, insurance card, driver’s license, etc.)
  • We will discuss your concerns during the counseling session.
Important Notes
  • Please note that first-time patients may have to wait longer before their examination.
  • We are required to verify your insurance card every month, so please be sure to bring it with you during your first visit of each month.
  • Please be advised that our clinic is not equipped with X-ray, abdominal ultrasound, or ECG facilities.
  • Cosmetic treatments are by appointment only. Please make a reservation in advance.
